Service Description

Medical history and physical exam will be completed on the day of your appointment. Bring all vaccines and medical records to your appointment. Screening laboratory tests will be required, and are not included in this cost. They may be performed before your appointment. If they are not, an additional visit will be necessary to complete the paperwork. Additional immunizations may be required, and are not included in this cost.
